Athletic Eligibility Forms

ATHLETIC ELIGIBILITY PACKET INFORMATION:
Please read before registering or completing any paperwork!

Registration Process Information

2017-2018 Sport Registration opened on Saturday July 1, 2017.

Spring Sport Registrations will close Friday February 2nd.

All paperwork must be turned in at that time. No later packets will be accepted.

Paperwork Turn In Dates

On these dates, we will open the media center to all who need to register. We will have a notary present and will approve all paperwork on site. This will alleviate communicating back and forth with the parents regarding missing items. The only item you need to bring is a physical (page 4 of the Athletic Eligibility Packet).

2017-2018 Dates:

Thursday January 18th- 5pm-6pm

  • Forms are valid until June 30th of each year and a new registration and packet, dated July 1 or after needs to be submitted in order to maintain eligibility!
  • Physicals are valid for 395 days. Packets are kept on file for the entire year and do not need to be re-submitted with a new packet.
  • The Weddington High School Athletic Department is pleased to partner with Family ID for online registration for the 2016-2017 school year! These forms must be completed prior to any involvement in pre-season skill development sessions or tryouts. Please click the box below to register.

Don't Get Caught Without a Valid Physical! The NCHSAA states that in order to be eligible for practice or participation in high school athletic activities a player must receive a medical examination once every 365 days by a duly licensed physician, nurse practitioner, or physician's assistant. If the athlete's physical expires they become ineligible and can not practice or play until a current physical is on file with the school. This includes in season and out of season workouts.
