ATHLETIC ELIGIBILITY PACKET INFORMATION:
Please read before registering or completing any paperwork!
2017-2018 Sport Registration opened on Saturday July 1, 2017.
Spring Sport Registrations CLOSED Friday February 2nd.
All paperwork must be turned in at that time. No later packets will be accepted.
On these dates, we will open the media center to all who need to register. We will have a notary present and will approve all paperwork on site. This will alleviate communicating back and forth with the parents regarding missing items. The only item you need to bring is a physical (page 4 of the Athletic Eligibility Packet).
We will not hold any more paperwork turn in dates for the 2017-2018 school year. Please submit all paperwork to the front office. A free notary is on site between the hours of 8am-3pm daily.
Once complete, please submit the Sports Eligibility Packet (found on the Athletics Forms Page) to the front office. The Athletic Eligibility Packet cannot be submitted until July 10, 2017 and must be dated July 1, 2017 or after. Once all paperwork is submitted and verified, an email will be sent to confirm eligibility.
For those just needing to update their physical:
If your physical has expired, you will only need to fill out and return the Pre-Participation AND Physical Form.
All physicals on or after July 1, 2017 MUST be on the attached forms. We will not accept any other forms!
You must attach a copy of the Coverage and Premium Application Summary AND the Student Insurance Card to your eligibility packet!
Don't Get Caught Without a Valid Physical! The NCHSAA states that in order to be eligible for practice or participation in high school athletic activities a player must receive a medical examination once every 395 days by a duly licensed physician, nurse practitioner, or physician's assistant. If the athlete's physical expires they become ineligible and can not practice or play until a current physical is on file with the school. This includes in season and out of season workouts.
